Age-old dilemma: A third of men aged 60 to 69, and 60 per cent of those aged over 70 found to have erectile problems in study

Healthy men who have trouble in the bedroom could be at risk of hidden heart disease, a study has found.

The link between erectile dysfunction and heart disease is well known, but the research showed that even in apparently healthy men, slight or moderate erection problems could signal trouble ahead.

The condition increased the risk of heart attacks, heart failure and arterial disease in men aged 45 and over with no previous history of heart conditions.

One expert commenting on the findings said men experiencing any degree of erectile dysfunction should seek urgent medical help.

Dr Rob Grenfell, from the Australian Heart Foundation which funded the study, said: 'These results tell us that every man who is suffering from any degree of erectile dysfunction should be seeking medical assistance as early as possible, and also insisting on a heart health check by their GP at the same time.'

Impotency is very common among middle aged and older men. Previous research has shown that around one in five men aged 40 and over reports moderate to severe erectile dysfunction.

Among the 95,000 men taking part in the new study for the Sax Institute in Sydney, this level of problem was experienced by 16 per cent of those aged 50 to 59, 34 per cent of those aged 60 to 69, and 60 per cent of those aged 70 and over.

Scientists looked at death rates among the participants and the number of hospital admissions for various heart and artery conditions over a period of two to three years.

During this time they recorded almost 8,000 admissions and 2,304 deaths.

Even among men with no previous record of heart disease, increasing severity of erectile dysfunction led to sharp rises in the risk of being hospitalised.

Mild erectile dysfunction was associated with a very small 0.08 per cent increased chance of an admission for ischaemic heart disease, a condition caused by blocked arteries. But this rose to a 37 per cent higher risk for men with moderate ED and 60 per cent for those severely affected.

Erectile problems had a major impact on heart failure admission rates, boosting them five-fold even for mild and moderate conditions.

Study leader Professor Emily Banks, said: 'The risks of future heart disease and premature death increased steadily with severity of erectile dysfunction ... in men with and without a history of cardiovascular disease.

'Rather than causing heart disease, erectile dysfunction is more likely to be a symptom or signal of underlying ‘silent’ heart disease and could in future become a useful marker to help doctors predict the risk of a cardiovascular problem.

'This is a sensitive topic but men shouldn’t suffer in silence; there are many effective treatments, both for erectile dysfunction and for cardiovascular disease.'

The scientists said ED could serve as a useful marker to identify men who should undergo tests to assess their risk of cardiovascular disease.

"The key message is that if you are willing to bring it up and discuss it with a healthcare professional, there are multiple alternatives to help you. A significant percentage of men will respond to therapies," Watkins-Bruner says. "The problem is that medical professionals tend not to deal well with any sexual dysfunction after treatment. Research has shown this. So it may be your job to take the initiative."

Watkins-Bruner emphasizes that men should tell their doctor about the problem as simply and directly as possible. "You can bring up the topic by saying something like, 'I have a concern. I have not been able to have an erection or have an erection as often.' Or you might say, 'I can have an erection, but it does not maintain itself.'" The most common side effects of these pills are headache, facial flushing, and upset stomach. Less common side effects are bluish or blurred vision and sensitivity to light, and a thorough eye exam is recommended before using these drugs. Men with diabetes are at a higher risk of erectile dysfunction or impotence, especially if their sugar levels are not well-controlled. So far, diabetes has been associated with complications of sight, heart and kidneys, but a new study shows potency complications in men with over 10 

years of diabetes. Researchers at west Delhi’s Maharaja Agrasen Hospital have studied over 200 diabetic men.

They found that at least 36% of the diabetics suffered from hypogonadotropic hypogonadism, meaning that sex glands were affected — leading to fertility complications.

“We estimate that 50-60% of men with prolonged diabetes suffer from erectile dysfunction. The study results support the belief of high prevalence of erectile dysfunction in men over 35 years of age as there is interference at brain level in the functioning of sex glands," said Dr Rajeev Chawla, a diabetologist, at the 8th Annual Conference of Research Society for the Study of Diabetes in India (RSSDI).

“We have felt the need to recommend the ADAM score test, which will help identify androgen deficiency in every diabetic patient over 40 years of age. Based on the score, one can also conduct free testosterone tests and sex hormone binding tests to identify erectile dysfunction problems. Hormonal imbalance can be corrected," he said. The study has also been sent to the American Diabetes Association for a conference in 2013.

"For many years, erectile dysfunction has been associated to with diabetes, but there were not many studies proving it. Now that we have data, we can infer that there is a correlation, but more data is needed to study the extent of damage," said Dr BM Makkar, senior diabetologist at RSSDI.

Genetic profile predicts postradiation erectile dysfunction

Scientists have identified a group of genetic markers that can predict the risk forerectile dysfunction (ED) after radiotherapy for prostate cancer.
The team says that the 12 single nucleotide polymorphisms (SNPs) each gave an odds ratio for ED of between 1.6 and 5.6 in a discovery cohort of 132 men with ED afterbrachytherapy and/or external beam radiotherapy and 103 men without this side effect.
The risk for ED increased significantly with the number of the SNPs carried, by a factor of 2.2 for each additional allele, after adjusting for patient ancestry and clinical factors.
When the pooled SNP profile was applied during treatment planning, alongside other risk factors such as age and treatment, the researchers were able to predict the likelihood of ED after radiotherapy with 84% sensitivity and 75% specificity in a validation cohort of 230 men, 128 of whom developed ED.
"The risk of developing erectile dysfunction after radiation treatment is highly variable, suggesting there may be a genetic component to determining that risk," commented author Barry Rosenstein (Mount Sinai School of Medicine, New York, USA) in a press release.
"Our study confirms that specific markers make certain patients more susceptible to this side effect."
Although none of the SNPs met the strict criteria for genome-wide significance, the researchers believe they may "represent true associations" because of their increased predictive power compared with other types of variable such as age, race, and body mass index.
"Notably, these 12 SNPs lie in or near genes involved in erectile function or other normal cellular functions (adhesion and signaling) rather than DNA damage repair," the team writes in the International Journal of Radiation Oncology Biology Physics.
The researchers suggest that further genetic analysis may lead to the identification of more predictive SNPs or rare variants that directly impact protein expression, modification, or transcription regulation.
